Community Living Specialist
Position Summary
The Community Living Specialist provides direct support, training, coordination, and ongoing services to Shared Living Providers and individuals receiving Independent Living and Supported Family Living services. This role ensures safe, effective supports and community resources are in place to help individuals thrive across all service settings.
This position works closely with providers, individuals, families, and community partners to promote stability, independence, quality care, and positive outcomes. The ideal candidate demonstrates leadership, professionalism, collaboration, strong communication skills, and a commitment to person-centered care.

Qualifications
Education
Bachelor’s degree in Social Work, Human Services, or related field required
Licensure/Certification
Valid driver’s license and auto insurance required
Experience
Minimum of two years of experience in Developmental Disabilities, Child Welfare, Juvenile Justice, and/or Mental Health Assessment and Treatment required
Preferred Skills
Intermediate computer skills including typing/keyboarding
Proficiency in Microsoft Word, Excel, and Outlook
Strong organizational, communication, and problem-solving skills
Ability to build positive relationships with individuals, families, providers, and community partners
Essential Responsibilities
Provider Support & Training
Conduct initial and ongoing training for Shared Living Providers
Identify provider training needs and gather feedback to improve support services
Offer individualized coaching and support tailored to participant needs within provider homes
Coordinate educational opportunities and resources with community partners
Participant Support & Advocacy
Provide crisis intervention, advocacy, and ongoing support to providers and participants
Maintain regular contact through home visits and on-call support
Attend Individual Support Plan (ISP) meetings and essential appointments
Deliver direct services focused on daily living skills, independence, and community integration
Assist participants in accessing community resources and maintaining stable living environments
Documentation & Compliance
Complete all required documentation accurately and on time
Conduct file audits and assist with Representative Payee financial oversight when applicable
Prepare materials for Rights Review Committee meetings
Ensure compliance with agency, state, and program requirements
Collaboration & Communication
Coordinate with families, providers, and support teams to assess needs and problem-solve challenges
Promote person-centered, respectful, and effective support services
Provide reliable transportation to home-based and community settings as needed
