Job Description:
We are seeking a skilled and compassionate professional to join our team as a Foster-Adoptive Care Case Manager. This individual will work closely with children, families, and community partners to provide case management services that promote the well-being and safety of children in foster care.
The ideal candidate will have a strong understanding of child development, parenting skills, and trauma-informed care. They will be responsible for conducting home visits, school visits, and attending Multi-Disciplinary Team (MDT) meetings and Individualized Education Program (IEP) meetings. Additionally, they will participate in on-call rotations and collaborate with other professionals to ensure comprehensive care and support for children and families.
Key Responsibilities:
- Conduct home visits and school visits to assess the needs of children and families
- Attend MDT meetings and IEP meetings to develop and implement treatment plans
- Participate in on-call rotations to provide emergency support and crisis intervention
- Collaborate with other professionals to ensure comprehensive care and support for children and families
Requirements:
- Bachelor's degree from an accredited college or university in a social service-related field
- Licensed by the West Virginia Board of Social Work Examiners or equivalent board
- Minimum two years of experience in children and family services, preferably with knowledge of the WV Medicaid Program
