PRIMARY CARE POSITION
Nurse Practitioner (NP) – Full-Time for Primary Care Position.
Location: Maryland (Multiple Rehabilitation Centers in Maryland)
Schedule: Flexible
We are seeking a dedicated and compassionate Nurse Practitioner (NP) to join our team and provide high-quality medical care to patients in post-acute and rehabilitation settings. This opportunity is available as either a full-time employed position or independent contractor role, offering flexibility while maintaining consistent patient care.
Full-Time Position (W-2) or 1099 (independent Contractor)
Salary: competitive salary based on experience

Responsibilities
- Provide routine and acute medical care to patients in post-acute and rehabilitation settings
- Document all patient encounters in the Electronic Health Record (EHR) daily
- Collaborate with interdisciplinary care teams to ensure continuity of care- such as Nursing and Physical Therapy
- Maintain professional, respectful relationships with patients, families, nursing staff, and SNF staff
- Adhere to facility policies and regulatory requirements
Qualifications
- Active, unrestricted Maryland Nurse Practitioner license, Maryland DEA License, and Maryland CDS License.
- Board Certified (Adult, Geriatric, or Family NP preferred)
- Experience in rehabilitation, long-term care, or geriatrics strongly preferred
- Strong documentation skills and EHR proficiency
- Excellent communication, interpersonal, and organizational skills
- Ability to work independently and manage time effectively
