PURPOSE

Comprehensive Community Support Services (CCSS) is a client-centered model that promotes rehabilitation, recovery, and resiliency. It is rooted in the belief that with support, individuals can address the barriers that interfere with the development of skills necessary for independent functioning in the community. The Community Support Worker (CSW) provides direct care services using various interventions and resources to empower clients to meet their goals. Operating in both the office and community, the CSW holds a caseload, works on an interdisciplinary team and is involved in crisis planning.

KEY RESPONSIBILITIES

  • Provide direct services in clinic and community settings to support clients with addressing functional deficits related to independent living, learning, working, socialization, and/or recreation 
  • Adheres to and implements Krossroads’ program models and court-monitored programming.
  • Maintain spreadsheets and documentation needed for contracts and reporting requirements. 
  • Build a collaborative relationship with upper management and development program tracking tools, measure program efficacy, maintain outreach resources list and active referral list.
  • Attend all clinical supervision, team meetings and employ directives from the leadership team.
  • Must be able to navigate electronic health record system - Advanced MD, have excellent digital literacy, organization, and planning skills.
  • Collaborate with the administrative team.
  • Ensure that CCSS services are at least 70% community-based
  • Engage with clients in the community to include using personal vehicle at times to transport clients.
  • The first 60 days of employment, build up and maintain a caseload 20 billable hours weekly that may include screening, assessments, developing care coordination plans, outreach and advocacy.
  • After 60 days of employment, caseload built to at least 28 billable hours
  • Support clients and co-workers in following Krossroads’ policies and procedures for managing crisis as needed 
  • Conduct urine drug screens (UDS)
  • Follow agency and State reporting protocols to include but not limited to critical incident reporting, internal incident reporting, mandated reporting of child abuse/neglect, and engaging safety protocols. 
  • Assist clients in accessing resources and coordinates wrap-around services in the community. 
  • CPSWs may lead educational/skills groups for clients in areas such as basic skills, mental health and addiction  
  • Assist clients to formulate and achieve goals.
  • Complete and update documentation including: CCSS treatment plan, crisis plan and functional assessment within required timeframes and according to Krossroads’ policies and procedures. 
  • Document progress notes daily completing all required areas in a timely manner.  
  • Works within an interdisciplinary team and engages in team meetings.  
  • CPSWs may lead educational/skills groups for clients in areas such as basic skills, mental health and addiction.
  • Maintains healthy boundaries with the client, and others that may be involved in their lives.

QUALIFICATIONS

  • Must be 18 years of age or older
  • Have a CCSS certification or acquire this certification within 90 days or hire.
  • Have reliable transportation and car insurance
  • Proficiency in Microsoft Word, e-mail, telemedicine, and electronic health record (EHR).
  • Preferred associate degree from an accredited university, and/or preferred experience in providing direct care to the client population.

Pay: $17.00 - $27.00 per hour
