Home Health RN – 13-Week Travel Contract
KPG Healthcare is seeking an experienced Home Health RN for a full-time 13-week travel assignment. The RN will provide skilled nursing care to patients in their homes, completing scheduled visits and managing care based on each patient's individual needs.
Local candidates also accepted at slightly lower premium rate.
Responsibilities:
- Provide skilled nursing care during in-home patient visits
- Assess patients and monitor changes in condition
- Develop and implement individualized plans of care
- Perform wound care, medication management, and other skilled nursing services as needed
- Complete OASIS assessments and required documentation
- Coordinate care with physicians, therapists, and other members of the healthcare team
- Document visits using Homecare Homebase (HCHB)
Requirements:
- Active RN license in the state of practice
- 1 year of recent Home Health experience
- OASIS experience required
- BLS certification (AHA)
- Homecare Homebase (HCHB) experience
- Associate's degree or higher
