The Social Worker provides direct social work services and counseling to residents/families/groups. Works to enhance psychosocial functioning, spiritual, cultural, emotional, financial and physical well-being of residents. Develops plan of intervention based on assessment and addresses the physical and psychological problems. Coordinates interventions. Assists residents/family by individual group education and team conference to help understand and follow recommendations. Performs interdisciplinary discharge planning and ongoing evaluation of documentation of plan Aware of changes in community resources and services/communicates this as appropriate. Assist with other department caseload as appropriate.
Requirements:
- 1 year experience in an acute care or behavioral health setting preferred
- Master’s Degree in Social Services Required
- Current BCLS upon hire
- MAB/CPI required within 60 days of hire and annually thereafter
