What are the responsibilities and job description for the RN Case Manager (PD-Days) position at KPC GLOBAL MEDICAL CENTERS INC.?
Collaborates with the patient and their families, the patient’s physicians, and all care providers in coordinating the care of the patient, utilizing skills of advocacy, communication and education. Identifies, facilitates and coordinates the appropriate services, resources, providers and facilities throughout the continuum of care.
Requirements:
- REGISTERED NURSE WTTH AT LEAST 5 YEARS ACUTE MEDICAL/SURGICAL NURSING EXPERIENCE AND 2 YEARS CASE MANAGEMENT EXPERIENCE
- MINIMUM 3 YEARS RN RECENT ACUTE CARE EXPERIENCE
- BASIC COMPUTER SKILLS
- KNOWLEDGE OF CASE MANAGEMENT COMPONETS: UTILIZATION REVIEW AND DISCHARGE PLANNING
- CA RN LICENSE & BLS CARD from American Heart Assoc.
