Basic Function
- Conduct welcome calls to new patients and introduce them to our home health services.
- Verify patient information and confirm their understanding of the care process.
- Address initial questions or concerns from patients and family members.
- Provide an overview of what to expect during their home health care experience.
- Ensure all onboarding documentation is completed accurately and in a timely manner.
- Communicate relevant patient details to nursing and administrative teams.
- Follow up with patients as needed to enhance their onboarding experience.
- Maintain detailed and accurate records of patient interactions.
- Coordinate care efficiently with ancillary companies and departments.
- High school diploma or equivalent required; associate or bachelor’s degree preferred.
- Previous experience in home health, healthcare administration, or patient services preferred.
- Excellent verbal and written communication skills.
- Compassionate, patient-focused, and professional demeanor.
- Strong organizational skills and attention to detail.
- Proficient in Microsoft Office and comfortable learning new software systems.
- Ability to work both independently and as part of a team in a fast-paced environment.
