Position Overview
We are seeking a skilled and motivated Home Health Field Registered Nurse (RN) to provide high-quality skilled nursing care to patients in their homes. The ideal candidate is reliable, organized, confident in clinical decision-making, and passionate about helping patients achieve their health goals.
Responsibilities
- Perform in-home patient assessments and skilled nursing visits
- Develop and update individualized care plans
- Perform wound care, disease management, medication administration, and patient education
- Communicate effectively with physicians, case managers, and interdisciplinary teams
- Complete accurate, timely documentation in accordance with regulatory requirements
- Participate in on-call rotation as needed
- Build trusting relationships with patients and families
Qualifications
- Current RN license in the state of Texas
- 1 year of nursing experience (Home Health preferred but not required)
- Strong clinical assessment and critical-thinking skills
- Reliable transportation, valid driver's license, and proof of auto insurance
- Comfortable using EMR/mobile charting
- Compassionate, professional, and patient-focused
