As a Care Coordinator, your role involves connecting individuals to services and resources best aligned to meet their unique needs. You'll undertake assessments, develop individualized care plans, oversee service coordination and referrals, and provide education to equip individuals with the tools to make informed, healthy lifestyle decisions. Opportunity Highlights:
Enhanced Care Management is a statewide Medi-Cal benefit available to individuals with complex needs. The Care Coordinator will lead the assessment, care plan development and ongoing care management of eligible participants. Our program specifically targets foster youth and other at-risk youth populations, but may also include their family members. The Care Coordinator utilizes motivational interviewing to engage individuals to support them in better managing their chronic conditions. The Care Coordinator actively engages individuals to remove barriers to health, social services, and behavioral health care.

This is a full-time, 40 hours per week position. The position is primarily remote, involving client visits in the field and occasional in-person meetings at the San Diego office.
Key Responsibilities:
Under the leadership of the ECM Supervisor, you will work with the ECM team to provide services to youth and families as follows:
- Conduct assessments of individuals' needs and formulate individualized care plans
- Enthusiastically educate and enroll clients about services, and motivate them to engage in services.
- Meet with individuals in their homes to facilitate services
- Oversee and ensure the seamless coordination of services, including physical, mental, dental care and social services
- Act as the primary liaison between individuals and their healthcare providers, and social services
- Stay informed about community resources and actively refer individuals to appropriate services as needed
- Maintain regular contact with clients to review and update care plans, and monitor clients' progress and outcomes
- Advocate for the needs and rights of clients within healthcare and social service settings
- Provide clients and families with resources and guidance on making informed health and lifestyle choices
- Document services according to ECM program standards
- Work closely with interdisciplinary teams, community partners, and stakeholders to enhance service delivery.
Qualifications:
- A strong desire to help people in need achieve successful outcomes
- Ability to speak Spanish preferred
- Minimum 2 years case management (or related) experience
- Knowledge of community resources
- Excellent interpersonal and communication skills
- Comfortable talking to people and motivating them to seek services
- Ability to collaborate with interdisciplinary teams
- Experience with high-needs populations
- Proficiency in electronic health records a plus
- A high degree of integrity, ethics and confidentiality
- Demonstrated ability to work independently and maintain productivity with minimal supervision
- Cultural sensitivity and awareness
- Valid driver’s license and auto insurance
We are seeking candidates who speak Spanish (preferred) and have at least two years of related work experience in social work, case management, community engagement or customer service. High school graduate or GED required. Prior experience as a community health worker, peer support specialist, medical assistant or similar role is a plus.
