Position Title: Program Coordinator - Complex Medical Help Program (Public Health Nurse)

This program serves individuals and families with serious or chronic health conditions by connecting them with medical, behavioral health, and community support services. The Program Coordinator plays a key role in ensuring participants receive the comprehensive care they need to improve health outcomes and quality of life.
Key Responsibilities
- Serve as the primary point of contact for program participants, families, and healthcare providers.
- Conduct intake assessments to identify medical, social, and behavioral health needs.
- Develop and monitor individualized care plans that address complex health challenges.
- Coordinate referrals to medical specialists, behavioral health providers, and community resources.
- Provide health education and support to promote disease management and prevention.
- Collaborate with internal teams, external agencies, and state program partners to ensure continuity of care.
- Track and document participant progress while ensuring compliance with state and program requirements.
- Prepare reports and program data for evaluation and quality improvement.
- Fatality Review board
- Early Childhood Coordinating Committee (EC3) member
- Licensed Registered Nurse (RN) in the State of Ohio, required.
- Bachelor of Science in Nursing (BSN) preferred; public health nursing experience a plus.
- Strong knowledge of chronic disease management, care coordination, and community health resources.
- Excellent communication, organizational, and problem-solving skills.
- Ability to work independently and collaboratively with multidisciplinary teams.
- Commitment to serving diverse populations with empathy and cultural competence.
- Full-time position based at Knox Public Health with community-based outreach.
- Some travel within Knox County may be required for home visits, partner meetings, etc.
