Responsibilities:
- Conduct thorough assessments of patients’ physical, emotional, and social needs within their home environments.
- Develop personalized care plans aligned with medical directives and social service resources to address each client’s unique circumstances.
- Administer medications, perform wound care, and monitor vital signs with precision and compassion.
- Educate clients and their families on health management, medication adherence, and wellness strategies.
- Maintain detailed documentation of care provided, progress notes, and communication logs in compliance with healthcare regulations such as Medicaid and Medicare standards.
Qualifications:
- Valid Registered Nurse (RN) license in good standing with current state requirements.
- Certification in CPR and First Aid
Additional Information:
- Agency vehicle is provided to complete daily visits and when on-call
- Hours are Monday - Friday 8am - 4pm
- 11.5 paid Holidays per year
- Background check and drug screening is required
Pay: From $30.00 per hour
