We are currently seeking an experienced and passionate Advanced Practice Provider to join our team of professionals dedicated to building a healthier Knox County.

The Knox County Health Department (KCHD) and Knox Community Health Center (KCHC) are proud to serve as a Federally Qualified Health Center (FQHC), providing comprehensive medical services to individuals and families across Knox County. Our integrated model of care ensures access to preventive services, chronic disease management, behavioral health, and patient education- all under one roof.

About the Role:

As an Advanced Practice Provider at KCHC, you will serve as a primary care provider responsible for delivering direct medical care to patients of all ages in a team-based setting. You will collaborate with physicians, nurses, behavioral health providers, and administrative staff to provide high-quality, coordinated care. This role also supports the clinical leadership and growth of the practice through quality improvement efforts and mentoring of clinical team members.

Key Responsibilities:

  • Perform comprehensive patient assessments, physical exams, and health screenings
  • Diagnose acute and chronic conditions and develop appropriate treatment plans
  • Prescribe medications, including psychotropics, and manage medication therapies
  • Monitor patient progress, adjust treatment as needed, and ensure appropriate follow-up
  • Refer patients to specialists or outside providers as clinically indicated
  • Provide leadership and guidance to medical support staff
  • Participate in quality improvement, risk management, and peer review activities
  • Maintain complete and timely clinical documentation in compliance with FQHC, billing, and regulatory standards
  • Participate in an on-call provider rotation as needed
  • Promote a culture of respect, compassion, and continuous learning within the clinic

Minimum Qualifications:

  • Current and valid Illinois license as an Advanced Practice Provider
  • National certification as applicable to discipline
  • Minimum of 3 years' experience in a clinical setting (FQHC or community health preferred)
  • DEA license and ability to prescribe medications in Illinois
  • CPR certification
  • Valid driver’s license and reliable transportation

Position Details:

This is a full-time contractual position, with compensation based on experience, number of hours, and credentials. KCHD employees participate in the Illinois Municipal Retirement Fund (IMRF) and are eligible to participate in Knox County Health, Dental, Vision, and Life Insurance. Salary ranges from $110,000 to $125,715 annually.
