JOB SUMMARY
The Director is responsible and supports the development, implementation and measurement of programs and projects related to value based care. This includes establishing, meeting, and continuously monitoring goals and objectives while maintaining alignment with the strategic goals and objectives of the organization. Responsible for the optimization of performance of value-aligned incentives, alternative payment models, quality measures and other population health metrics. The director works closely with a variety of stakeholders, coordinating and overseeing the activities of population health across the organization.
NOWLEDGE AND SKILLS
- Knowledge of value based, government and reimbursement regulation requirements
- Knowledge and experience in organizational effectiveness and operations management
- Knowledge of project management principles and practices
- Working knowledge of fiscal management principles and practices.
- Ability to manage, change, resolve conflict, problem solve and make decisions
- Information technology skills (Microsoft office suite and hospital based systems)

PRIMARY JOB RESPONSIBILITIES
- Directs, supervises and oversees activities of staff
- Provides leadership, direction and coordination in value based contracting, gaps in care and quality related to population health.
- Oversee overall financial management, planning, system and controls for supplies and accounts payable functions
- Formulates objectives, goals, and strategies collaboratively with other stakeholders
- Prepares and delivers reports to leadership outlining progress toward meeting annual goals and objectives.
- Creating annual budgets
- Remaining current of new trends and best practices and incorporates into industry standard
- Plan, organize, lead and coordinate all special projects for the department while excelling in communication and decision making
- Problem solve and delegate where appropriate
ADDITIONAL RESPONSIBILITIES

- Maintain professional growth for self to ensure current and appropriate knowledge base or area of responsibility.

EDUCATION AND WORK EXPERIENCE
- Bachelors required / Masters preferred
- Minimum of 3-5 years’ experience in Population Health, quality or process improvement. 2 years leadership experience.
- Preferred Population Health Certification or Master’s Degree.
