Kline Galland Hospice is looking for a compassionate and experienced Hospice Social Worker to join our team on a
On-call WEEKEND (Saturday and Sunday) basis. This flexible role is ideal for professionals seeking meaningful work while maintaining a balanced schedule.
Responsibilities:
- Conduct comprehensive psychosocial assessments for hospice patients.
- Collaborate with the interdisciplinary team to develop individualized care plans.
- Provide crisis intervention and therapeutic support to patients and families.
- Coordinate community resources and referrals.
- Deliver services in private homes, Adult Family Homes, Assisted Living, Independent Living, and Skilled Nursing Facilities.
- Master’s Degree in Social Work (MSW), Marriage and Family Therapy (MFT), or Mental Health Counseling (MHC) from an accredited institution.
- Minimum of 2 years of social work experience (1 year in Hospice or Home Health preferred).
- Strong verbal and written communication skills.
- Proficiency in electronic charting and documentation.
- Valid driver’s license, auto insurance, and reliable vehicle required.
