Kline Galland Benaroya Services is seeking a compassionate, experienced Hospice Inpatient Care & Transitions Clinical Specialist (RN) to support patients and families during one of the most meaningful moments in care. This role serves as the primary clinical representative for hospice within hospital and inpatient settings—providing evaluations, managing GIP admissions, coordinating transitions, and educating providers, patients, and caregivers.

The Hospice Transitions Clinical Specialist (Inpatient Focus) is responsible for evaluating hospice eligibility at the bedside, completing General Inpatient (GIP) admissions, coordinating transitions to home or facility-based hospice, and serving as a trusted resource to hospital partners. This RN ensures timely, compassionate, compliant hospice processes and strengthens relationships across providers and care settings.


Key Responsibilities

Patient Evaluation & Eligibility

  • Perform bedside hospice evaluations within hospital and inpatient settings.
  • Determine eligibility using CMS guidelines, clinical judgment, and organizational criteria.
  • Collaborate with attending physicians, hospitalists, and care teams to confirm hospice appropriateness.

Care Coordination & Admission Management

  • Complete the full admission process: consent, assessment, documentation, and communication for GIP care.
  • Coordinate transitions to GIP or routine home care as appropriate.
  • Support smooth discharges to home hospice, including equipment, medications, and communication with field teams.
  • Coordinate transitions to hospice services within Skilled Nursing Facilities (SNFs).
  • Respond rapidly to referrals to support symptom management and timely placement.

Education & Relationship Building

  • Educate hospital staff, case managers, and physicians on hospice philosophy and levels of care.
  • Serve as the liaison between hospital partners and hospice leadership.
  • Provide compassionate education to patients and families about hospice services and goals of care.
  • Facilitate presentations, in‑services, and educational sessions for internal and external partners.

Communication & Collaboration

  • Maintain timely communication with hospice intake, nursing, social work, and leadership.
  • Participate in interdisciplinary discussions regarding patient care plans.
  • Advocate for patients requiring symptom management or end-of-life support.
  • Collaborate with the liaison team on coverage, referral insights, and workflow improvements.

Patient & Team Support

  • Provide support to hospice triage as needed.
  • Assist field clinicians during high-volume periods, urgent admissions, or complex situations.

Compliance, Quality & Documentation

  • Ensure accurate, compliant documentation per EMR, CMS, and organizational standards.
  • Support chart audits and quality/compliance initiatives.
  • Stay current on Medicare hospice benefit requirements.
  • Follow all HIPAA, CMS, and organizational policies.

 

Qualifications:

 

  • Current Washington state Registered Nurse (RN) license or Multistate license.
  • Minimum 5 years RN experience preferred.
  • At least 2–3 years in hospice, palliative care, critical care, case management, or related field.
  • Strong understanding of hospice eligibility, GIP level of care, and hospice philosophy.
  • Exceptional communication and relationship‑building ability.
  • Ability to work independently in fast‑paced hospital environments.
  • Reliable transportation for travel between assigned hospitals and facilities.

Working Conditions

  • Work primarily in hospitals and inpatient facilities.
  • Travel to patient homes, assisted living communities, adult family homes, and SNFs.
  • Frequent bedside interaction with patients and families.
  • Requires mobility throughout hospitals and community settings.
  • Ability to sit, stand, walk, and drive for extended periods.
