Schedule: Tuesday to Friday 10 hour shifts
We are seeking a dedicated and experienced RN Registered Nurse to join our hospice team as a Full time Facility-Based Hospice Case Manager and includes a $5,000 Signing Bonus. This role plans, organizes and directs care for Hospice patients at Kline Galland’s Skilled Nursing Facility (South Seattle). The RN Case Manager is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community and facility to collaborate, coordinate, plan and direct services to meet the needs of individual and families within their homes and communities. As a non-profit 501(c)(3), Kline Galland employees are eligible for the Public Service Loan Forgiveness Program. Offering relief on student loans!
A few of the Essential Job Functions/Responsibilities
-Completes new patient Admissions initial assessments and comprehensive and ongoing comprehensive assessments of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es).
-Supports symptom and care management for all hospice patients at Kline Galland Home skilled nursing facility on scheduled days of work as needed.
-Collaborates with facility staff and leadership to support hospice patients within the regulatory requirements of the skilled nursing facility.
-Attends regular hospice team meetings and clinical rounds for patient review at the skilled nursing facility.
-Provides professional nursing care by utilizing all elements of nursing process.
Qualifications
- Current Registered Nurse (RN) license in WA or Multistate license
- Minimum of 1 year of experience in hospice or palliative care.
- Strong clinical assessment and care planning skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Compassionate and empathetic approach to patient care.
- Proficiency in electronic medical records (EMR) systems.
Salary : $45 - $65
