Duties

- Provide education and support to patients and their families regarding health management and available resources.
- Coordinate services across various settings including senior care, hospice care, and home care.
- Supervise nursing staff and manage team performance to ensure high-quality patient care.
- Maintain accurate documentation of patient interactions, assessments, and interventions.
