Job Overview

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our long-term care team. In this vital role, you will provide high-quality nursing care to residents in a skilled nursing or assisted living environment, ensuring their comfort, safety, and well-being. Your expertise will support the health needs of our residents, including those with complex medical conditions such as dementia, Alzheimer's, and other chronic illnesses. Responsibilities

• Administer medications and treatments accurately following physician orders, including injections, IV infusions, and catheter care.
• Conduct comprehensive patient assessments by monitoring vital signs, evaluating symptoms, and documenting medical histories using Electronic Health Record (EHR) systems.
• Assist with activities of daily living such as mobility support, tube feeding, wound care, and physical examinations to promote resident independence and comfort.
• Collaborate with interdisciplinary teams to develop and implement individualized care plans tailored to each resident’s needs.
• Ensure compliance with long-term care regulations, HIPAA confidentiality policies, and infection control protocols while maintaining meticulous medical documentation.
• Provide emergency response support including first aid, airway management, acute pain management, and triage for urgent health issues.
• Communicate effectively with residents’ families and caregivers to update them on health status and coordinate ongoing care strategies.

Skills

• Proven experience working with geriatric patients in settings such as skilled nursing facilities, assisted living communities, or long-term care environments.
• Strong knowledge of medical terminology, physiology, ICD coding (International Classification of Diseases), and medication administration procedures.
• Familiarity with EMR/EHR systems like Epic or Cerner for accurate documentation and patient record management.
• Skilled in performing clinical procedures including venipuncture, catheterization, sterile processing techniques, and specimen collection/processing.
• Ability to handle complex cases involving dementia care, Alzheimer's disease management, hospice care considerations, and behavioral health challenges.
• Experience working with individuals with developmental disabilities or special needs populations is highly desirable.
• Excellent communication skills for caregiver interaction, patient assessment, case management, and family education.

Duration: 13 weeks

Shift: Nights 12*3

Schedule: 6:00 PM - 6:30 AM
