CASE MANAGER – PROGRAM SPECIALIST

Reports To: Executive Director
Program Oversight Coordination With: Program Manager
Compliance Coordination With: Data & Compliance Officer
FLSA Status: Non-Exempt
Position Summary
KLCCOA is seeking a skilled and compassionate Case Manager to provide client-centered services to older adults and veterans. This position is responsible for eligibility determination, needs assessment, service coordination, and compliance-driven documentation in alignment with the Older Americans Act (OAA) and Oregon DHS/APD requirements.
Program Allocation:
- 50% Veteran-Directed Care (VDC)
- 25% Oregon Project Independence – Medical (OPI-M)
- 25% Additional programs
What You’ll Do
- Conduct intake and eligibility screenings
- Perform comprehensive needs assessments
- Develop and monitor service plans
- Coordinate services with community partners
- Maintain accurate, timely, and compliant documentation
- Manage assigned caseload and ensure continuity of care
Preferred Qualifications
- Bachelor’s degree in Social Work, Human Services, Psychology, or related field
- 2 years of case management or care coordination experience
- Experience working with older adults, veterans, or individuals with disabilities
- Familiarity with Medicaid, OPI-M, or Veteran-Directed Care programs
- Ability to manage complex caseloads independently
