Nurse Practitioner – Dementia & Geriatric Specialty

Practice at the top of your license, with a sustainable caseload and the time to actually care for your patients. At Kitwood Health, your clinical judgment is trusted, your schedule is realistic, and your work centers on building lasting relationships with older adults living with cognitive impairment — and the families who love them.

Kitwood Health partners with primary care practices. You would be working in the primary care setting alongside the primary care provider to manage and deliver comprehensive dementia care: early diagnosis, thoughtful planning, caregiver support, and longitudinal follow-up across the disease course.

What you can expect

  • Autonomous, NP-led practice with physician collaboration and specialty support
  • Daytime, weekday schedule — rare after-hours asks
  • Hybrid role with meaningful work-from-home flexibility
  • Longer visits, real continuity — quality over volume
  • Dedicated care coordinators and dementia-specific workflows
  • Mentorship in dementia and palliative care, with growth paths into leadership and program development

What you'll do

Provide patient-centered care across the full arc — from MCI to advanced dementia — as a trusted clinical partner through diagnosis, progression, and care transitions:

  • Conduct cognitive evaluations, order/interpret diagnostics, and deliver diagnoses with clarity and compassion
  • Build and manage individualized care plans, partnering with caregivers on behavioral and symptom management
  • Assess function and safety (ADLs, mobility, fall risk) and recommend supportive services
  • Coordinate across primary care, neurology, psychiatry, and community resources as clinical escalation point
  • Educate and support caregivers through disease progression and crisis moments
  • Lead advance care planning — goals-of-care conversations, directives, and proactive crisis prevention

Qualifications

  • Active APRN, PA or RN licensure
  • 3 years in geriatrics, neurology, dementia care, or related field
  • PMHNP, AGNP, GS-C, or similar certification preferred
  • Skilled at longitudinal care, caregiver dynamics, and end-of-life conversations
