***This is a full time position working rotating weekends on our Memory Care floor*** Previous activities experience in an assisted living facility is strongly desired
Schedule is 8:30A-5P on rotating days :
Mon, Tues, Wed, Sat then
Sun, Mon, Wed, Thurs, Fri
This position rotates with another Activity Coordinator from resident activities and directing Hillside visitors in the lobby.
PURPOSE OF POSITION:
Under the guidance and supervision of management staff, serves as a member of the Specialty Care Living team responsible for activities for those residents with dementia on the Memory Care Unit.
MAJOR RESPONSIBILITIES:
Serves as a member of the care team responsible for assisting and encouraging residents both on and off the unit
Responsible for listing residents who leave the floor to attend activities and communicating with the nursing supervisor assuring that she/he is aware of which residents will be off the floor or out of the facility.
Responsible for assessing that there is the correct number of staff required to safely transport residents and communicate this to the nursing supervisor or Unit Coordinator
Responsible for planning and implementing activities on the unit when residents are not involved in house wide activities, both group and one on one activities
Communicate with families via email, sending updates on resident participation and involving them in the activities on the unit.
Must be able to drive the bus and transport residents to and from activities when required
Responsible for documentation of activities attended by all residents
Responsible for organization and maintenance of the activity closet, communicating any needs to the nursing supervisor or Unit Coordinator
Communicate regularly with the Activity Director regarding calendar programs and needs of the residents
Assist RCA’s with meals as needed to allow them a break
Responsible for acting as an RCA as needed when the unit is short staffed
Must be willing to adjust hours as necessary to cover all activities as planned and resident needs
Assist is miscellaneous departmental duties/functions as needed
Responsible for complying with policies regarding health and safety in the home environment
Assists in miscellaneous departmental duties/functions as needed
All other duties as assigned
EDUCATION, EXPERIENCE, AND SKILLS REQUIRED:
High school diploma/GED
Two plus years direct applicable industry experience preferred
After employment Successful Completion of Dementia Care Course and Exam Certification preferred
Excellent verbal communication skills
Must be able to work with team members and work with minimal supervision
Computer skills, using Microsoft Office required
PHYSICAL REQUIREMENTS:
Lift up to 50 pounds
