About The Role
Kintegra Health is seeking a compassionate Licensed Dietitian or Registered Nurse with a passion for improving the lives of adults living with diabetes and other chronic illnesses. As a Chronic Illness Patient Educator, you’ll play a key role in supporting patients through individualized and group-based education — empowering them to manage their conditions, make sustainable lifestyle changes, and achieve better health outcomes.
This role combines direct patient care, health education, and community engagement, with opportunities to lead diabetes self-management and weight management programs across Kintegra’s service area.
Key Responsibilities
- Deliver 1:1 patient education and care focused on Diabetes Self-Management Education and Support (DSMES) and Medical Nutrition Therapy (MNT).
- Assess, plan, and implement individualized care plans for chronic conditions such as Type 2 Diabetes, obesity, dyslipidemia, and hypercholesterolemia.
- Facilitate group classes and workshops that meet American Diabetes Association (ADA) standards, helping maintain Kintegra’s recognition as an accredited education site.
- Provide weight management counseling for adults with unhealthy BMI levels and associated conditions.
- Collaborate with community partners to promote public health initiatives and participate in local health events.
- Maintain accurate and timely documentation in the Electronic Medical Record (EMR).
- Understand and apply basic billing practices relevant to RD/CDE services.
- Participate in continuous quality improvement activities to enhance patient care delivery.
- Protect patient confidentiality in full compliance with HIPAA standards.
- Travel between Kintegra Health sites and community events as needed.
Education
- Bachelor’s degree preferred.
- Minimum of 2 years providing outpatient Medical Nutrition Therapy.
- Minimum of 2 years providing Diabetes Self-Management Education and Support (DSMES), with a focus on chronic illness and disease prevention.
- RN (with at least 2 years’ experience in Diabetes Management) or Licensed Dietitian (NC/LD).
- BLS certification required.
- RD or CDCES certification preferred but not required.
- Valid driver’s license and reliable transportation.
- Ability to work independently with minimal supervision.
- Strong communication and interpersonal skills for effective patient and team interaction.
- Proficient in EMR systems, Microsoft Word, and Excel.
- Must be able to sit, stand, bend, and lift up to 50 pounds as needed.
- Ability to read, write, and communicate clearly in English.
