This is a contract role for a Physical Therapist specializing in outpatient care at home. Located in Manhattan, NY, this is an on-site position involving visits to patients’ homes. Responsibilities include conducting evaluations, developing personalized therapeutic care plans, providing one-on-one rehabilitation services, and ensuring patient progress through quality care. Therapists will collaborate with patients to achieve mobility goals and improve their overall quality of life.
- Proficiency in Physical Therapy and Rehabilitation techniques
- Strong foundation in Medicine and Patient Care practices
- Experience delivering Home Care services, with a focus on tailored, individualized plans
- Current state licensure as a Physical Therapist in New York
