Responsibilities
- Monitor and assess patient conditions during overnight shifts, including vital signs, physical examinations, and symptom evaluation.
- Administer medications and injections accurately following established protocols, including IV infusions and aseptic techniques.
- Document all patient interactions, assessments, treatments, and medication administrations clearly within electronic health record (EHR) systems, PCC
- Assist with complex procedures such as catheterization, dialysis management, and wound care including ostomy care.
- Collaborate with interdisciplinary teams on resident matters
- Ensure infection control standards are maintained through proper specimen collection/processing and adherence to HIPAA regulations for medical records confidentiality.
Skills
- Skilled in working with diverse patient populations including geriatrics, individuals dementia care needs.
- Proficient in EMR/EHR systems like Point Click Care; capable of detailed medical documentation and medical records management.
- Demonstrated ability to perform laboratory procedures such as phlebotomy and specimen collection; experienced in IV insertion and catheter care.
- Excellent patient assessment skills with experience in triage, urgent care protocols, hospice & palliative medicine or post-acute care settings.
- Knowledgeable about infection control practices, aseptic techniques, airway management, and emergency response procedures including basic life support (BLS).
