Kings View is a nonprofit leader in providing behavioral health services to the underserved community, is currently seeking a driven Crisis Case Manager II to join our mission to provide compassionate services to those with limited resources. The Crisis Case Manager II is a key member of the 24/7 Mobile Crisis Response Team, providing support to individuals of all ages in mental health crises. With minimal supervision, the Case Manager responds to various non-life-threatening calls without police or EMS. They conduct intake and needs assessments, coordinate services, refer clients to treatment resources, and advocate for ongoing care. Acting as a lead for Crisis Case Manager I's, the Crisis Case Manager II provides guidance and support to ensure effective service delivery. With strong outreach skills and knowledge of community resources, they help resolve crises, de-escalate situations, prevent hospitalization, and promote stability in the community.

How will I contribute

  • •    Provide non-clinical care coordination services to individuals with serious mental illness (SMI) and substance use disorders (SUD).
    •    Assist clients who have received crisis intervention in developing post-crisis engagement plans in collaboration with the Crisis Mobile Response Team.
    •    Complete all documentation within established time frames.
    •    Act as a liaison between clients, their families, and the Crisis Mobile Response Team.
    •    Communicate important information to the supervisor or designee, both in writing and verbally.
    •    Transport clients when approved by the supervisor.
    •    Offer linkage and consultation services related to community resources, accompanying clients when necessary for warm hand-offs.
    •    Collaborate with the Crisis Mobile Response Team to help prevent re-hospitalization and promote stability.
    •    Work flexible hours, including weekends and evenings, to meet program needs.
    •    Support clients in navigating agency referrals to access resources and prevent re-hospitalization.
    •    Maintain accurate clinical records following agency standards, including COA, HIPAA, CCBHC, and CalAIMS, and ensure timely documentation, including required signatures.
    •    Conduct follow-up check-ins with clients within 72 hours after the initial crisis response.
    •    Develop and maintain a Care Coordination Resource Binder.
    •    Assist in accessing mental health services and community resources.
    •    Implement brief life skills coaching and develop compliance plans for clients who have missed appointments.
    •    Maintain accurate records of post-crisis follow-up, including linkage efforts after hospitalization.
    •    Coordinate with community partners to identify and participate in outreach events.
    •    Complete four hours of cultural competency training each fiscal year.
    •    Achieve advanced proficiency in the electronic record-keeping system, including effective use of SmartCare and One Point for documentation and timekeeping.
    •    Provide leadership and guidance to Crisis Case Manager I's to ensure effective service delivery and team support.
    •    Participate in on-call services on a rotational basis as required by the program.
    •    May encounter risk of working with persons with SMI (serious mental illness) that may include, but not limited to persons in mental health crisis, persons that may be intoxicated or impaired by drugs or alcohol, persons who may be DTO (danger to others), DTS (Danger to Self), or GD (Gravely Disabled). If such situations arise, staff will be required to call local emergency personnel to help deescalate the crisis, use crisis intervention techniques, and consult with supervisor as soon as practically possible.
    •    Perform other duties as assigned to support the overall functioning and success of the program.

 

What I bring

  • Associate Degree
  • 4 years’ experience working with the un-housed, mental health and substance use, or related populations. Advanced experience may be substituted in lieu of college degree on a year-for-year basis.
  • Class C Driving License Required
  • Solid understanding of working within a harm reduction framework to promote safety and well-being for individuals in crisis, along with knowledge of the cultural and socio-economic factors that influence service delivery for those with mental health and co-occurring disorders. Familiarity with community resources is also essential, as these resources play a critical role in supporting clients and enhancing their recovery journeys.
    Effective data entry skills using various electronic systems while maintaining the integrity and accuracy of the data. Strong verbal and written communication skills to collaborate with person served family members and friends as well as other professionals linked to ongoing care and ability to maintain good case records, including documentation and safety planning. Critical thinking and problem solving with the Mobile Crisis Response Team to determine the best care plan for the person served. Ability to react appropriately to emergency situations. Ability to work independently and effectively collaborate as part of a team.
    Action oriented. Strong problem-solving skills. Excellent organizational skills, ability to multitask and prioritize multiple tasks. Outstanding ability to follow-through with tasks, while remaining flexible and reliable. Compassionate and empathetic.
