Under minimal supervision, as an integral part of a treatment team, the Case Manager I acts as a liaison between treatment providers and the client. The Case Manager I is responsible for intake and needs assessment, arrangement and coordination of comprehensive services, referral to treatment and recovery services, monitoring and evaluation of progress and changing needs and advocacy. The Case Manager I offer their community outreach abilities and extensive knowledge of community resources and support as a primary means of assisting the crisis team in efforts to prevent re-hospitalization and to promote stabilization.

 

How will I contribute

 

  • Provide various non-clinical Case Management services to SMI/SED s.
  • Assist clients who were provided crisis intervention to develop a post-crisis/OP engagement plan in coordination with the Access/Crisis Team.
  • Complete all documentation within same day as service delivery, at a frequency of 80% or better.
  • Act as liaison between client/client’s family and crisis response team.
  • Communicate pertinent information, both in writing and verbally to supervisor or Designee.
  • Transport clients when approved by supervisor.
  • Provide linkage and consultation services related to community resources, requests for general information about our services, and in response to client requests for access to services (i.e., screenings).
  • Work closely with the crisis team day shifts in team efforts to prevent re-hospitalization and to promote stabilization.
  • Weekend and evening hours will be required to meet program needs.
  • Assist clients in the navigation of agency referrals that could provide resources to prevent re-hospitalization. 
  • Familiarity with and adherence to compliance with the code of conduct and the requirements of the compliance program are elements of performance evaluations and conditions of employment.
  • Review crisis roll-over emails daily to identify individuals in need of post crisis follow up services and provide linkage to mental health services, and other community resources, by making a minimum of 3 contact attempts within a 7-day period.
  • Develop and maintain Resource Binder. 
  • Support Access functions by conducting screenings on scheduled days and as needed.
  • Assist Clients in accessing mental health services by completing open access screenings using all relevant screening tools.
  • Develop and assist in the implementation of brief life skills coaching of individuals/s, linkage to wraparound services/resources, and compliance planning for no-shows for medication management services.
  • Assist clients in the development of compliance plan when they have no-showed/late canceled for an initial doctor’s appointment. 
  • Maintain accurate and timely records of post crisis follow up and open access screenings by updating post crisis and screening logs. Maintain detailed log(s) related to post-hospitalization linkage efforts. 
  • Coordinate with community partners to identify and attend various outreach events.
  • Achieve four hours of cultural competency training each fiscal year.
  • Achieve advanced competence in using our electronic record keeping system. Includes but is not limited to the following: a. Use of SmartCare; b. Effective documentation of services provided; c. Competence in using assigned laptop at remote locations; d. Effective use of One Point, the electronic timecard system.
  • May encounter risk of working with persons with SMI (serious mental illness) that may include, but not limited to persons in mental health crisis, persons that may be intoxicated or impaired by drugs or alcohol, persons who may be DTO (danger to others), DTS (Danger to Self), or GD (Gravely Disabled). If such situations arise, staff will be required to call local emergency personnel to help deescalate the crisis, use crisis intervention techniques, and consult with supervisor as soon as practically possible.
  • Perform other duties as assigned.

 

What I bring

 

  • High School Diploma (or Equivalent)
  • Driver License: Class C
  • 2 years’ experience working with the homeless, mental health, substance abuse population, or related.
  • Knowledge of how to work effectively in harm reduction environment. Knowledge of cultural and socio-economic factors that inform service provision to persons with mental health and cooccurring disorders, and community resources that will be utilized by clients.
  • Effective data entry skills using various electronic systems while maintaining the integrity and accuracy of the data. Strong verbal and written communication skills to support family members and friends and professionals the case and care plan and maintain good case records. Critical thinking and problem solving to determine the best care plan for each  after assessing s, analyzing notes from health care and social workers. Ability to react appropriately to emergency situations. Ability to work independently and effectively collaborate with a team.
  • Action oriented. Strong problem-solving skills. Excellent organizational skills, ability to multitask and juggle multiple priorities. Outstanding ability to follow-through with tasks.  Flexible and reliable. Compassionate and able to relate to different s with various needs.
  • Bachelor's degree in behavioral science or related. Bilingual in Spanish preferred.
