Kingdom Builders Transitional Housing Program (KBTHP) provides a co-ed, integrated, trauma-informed 24/7 residential transitional program for TAY (Transitional Age Youth, 18-25) providing meals, support services, resources, programming, and supervision in a safe, clean, drug-free environment. The KBTHP focuses on a compilation of services that addresses each individual participant’s needs with one specific goal; to assist residents with life skills and successful integration into the community.

Position Title: Case Manager (FTE & Part-time available)

Compensation: Commensurate with Experience

Status: Hourly

Position Overview: As a foundingCase Manager, you will play a critical role not only in providing direct support to youth residents, but also in shaping the program’s culture, systems, and services. This is an exciting opportunity for a mission-driven professional to help design and implement a trauma-informed, equity-centered model of care. The Case Manager will provide individualized, strengths-based support to young people navigating housing instability, system involvement, and other complex barriers to independence. This role is ideal for someone who thrives in collaborative environments, believes deeply in the leadership potential of youth, and wants to be part of building something transformative from day one.

Key Duties: Co-build program implementation and culture-building, Trauma-informed, Youth-Centered case management, Promote safety, boundaries and conflict resolution in co-ed housing, Wraparound Services & Workshop Coordination.

Program Launch Date: October 2025 | Position open until filled with the ideal candidate

Responsibilities include but shall not be limited to the following:

  • Conduct comprehensive intake and orientation for new residents, completing bio-psycho-social assessments and determining needs for medical, therapeutic, or psychiatric services.
  • Develop, implement, and regularly update Individualized Service Plans (ISPs) that set clear schedules, routines, and interdisciplinary service coordination for a caseload of 15–17 Transitional Age Youth (TAY).
  • Provide weekly 1-to-1, trauma-informed case management sessions, verifying attendance, tracking goals, and adapting supports based on progress and observed responses.
  • Facilitate access to wraparound resources (mental-health, substance-use, education, employment, legal, benefits) and arrange logistics such as transportation or escort as needed.
  • Monitor resident safety and community dynamics in the co-ed housing environment, promoting healthy boundaries, conflict resolution, and restorative practices.
  • Lead or co-lead life-skills and wellness workshops that reinforce ISP goals and build resident capacity for independent living.
  • Maintain accurate electronic case notes, assessments, and documentation in various case management database systems; compile statistics and prepare scheduled program, outcomes, and funder reports.
  • Communicate resident progress through weekly case meetings, sharing results, obstacles, and recommended adjustments with the interdisciplinary team.
  • Respond to crises and participate in the on-call rotation, advocating for needed services and intervening to stabilize situations.
  • Contribute to continuous program design and culture-building, evaluating and refining processes, rewriting procedures, and implementing improvements that enhance treatment results.
  • Advance professional and team competence by pursuing training, sharing learning resources, and mentoring peers when appropriate.
  • Represent Kingdom Builders positively by embracing new tasks, proposing innovative solutions, and upholding resident confidentiality and organizational values at all times.

Qualifications:

Required:

  • Experience or degree in Social Work, Psychology, Human Services, or a related field; equivalent professional or lived experience considered in lieu of a degree
  • At least 3 years of experience working with Transitional Age Youth (TAY) or similar populations impacted by housing instability, foster care, or justice involvement
  • Strong understanding of trauma-informed care, harm reduction, and youth-centered approaches
  • Excellent interpersonal, communication, and conflict-resolution skills
  • Experience developing service plans, linking clients to community-based resources, and maintaining professional documentation
  • Ability to navigate a fast-paced, collaborative, and culturally diverse environment
  • Proficiency in basic computer applications and electronic case management systems
  • Ability to work some evenings, weekends, and participate in on-call rotation
  • Must pass background check

Preferred:

  • Master’s degree in Social Work (MSW) or related field
  • Bilingual in Spanish
  • Familiarity with Alameda County housing and social service system
  • Lived experience of homelessness, system involvement, or incarceration strongly encouraged

Physical Requirements:

  • Ability to remain in a stationary position (e.g., at a desk or computer) for extended periods
  • Ability to move about the facility, including climbing stairs and accessing units on multiple floors
  • Must be able to occasionally lift or carry up to 20 pounds (e.g., supplies, food boxes, resource kits)
  • Capacity to respond quickly to on-site emergencies or resident needs
  • May require occasional travel within Alameda County for meetings, outreach, or accompaniment to services
