As our most acute level of care,Kindred'stransitional care hospitals offer the samecritical carepatients receive in a traditionalhospitalorintensive care unit, but for an extended recovery period. Ourcliniciansplay a vital role in the recovery process forchronic, critically ill and medically complex patients.
As aSocial Workerwithin our Sub Acute Unit you will:
- Provides services to respond to the emotional needs of patients and their families.
- Provides psychosocial assessments, intervention, and/or treatment and discharge planning to patients and families to assist them in coping with their hospitalization, illness, diagnosis, and/or life situation and meet the continuing needs of patients post hospitalization.
- Assists patients and families to resolve personal financial and environmental difficulties which interfere with the care management process. Provides crisis intervention, individual and family therapy where skilled social work judgment is required.
- Directs access to appropriate community and adjunct resources that foster improved quality of life.
- May also assist with patient admission and referrals.
As aSocial Workerwithin our Sub Acute Unit you will have:
- Bachelor's Degree in Social Work from an accredited school of Social Work or Bachelor Degree in a human services field preferred
- Certificate as required by the state of employment.
- 1 year of supervised experience in long-term or sub-acute care.
- Management/administrative experience preferred.
