Duties
- Provide direct support and assistance to clients in their own homes or community settings, ensuring their safety and well-being.

- Facilitate access to community resources such as healthcare, education, employment, and social activities.
- Assist clients with daily living activities including mobility, medication management, and household tasks.
- Monitor client progress and document interactions accurately to ensure consistent quality of care.
- Collaborate with families, caregivers, and multidisciplinary teams to coordinate services effectively.
- Promote independence by empowering clients through skill-building activities and motivational support.
Experience
- Prior experience working in community outreach, social services, or related fields is highly desirable.
- Strong interpersonal skills with the ability to build trusting relationships with diverse populations.
- Excellent communication skills—both verbal and written—to effectively document and report client progress.
- Ability to work independently while managing multiple priorities in a fast-paced environment.
- Knowledge of local community resources and social service programs is a plus.
