Kindred Community Cares is a new start-up multi-specialty clinic providing primary care and behavioral healthcare. We are preparing to relocate in order to expand our services. Duties
- Conduct thorough initial intake and follow-up physical examinations and patient assessments, including vital signs, medical history reviews, and physiological evaluations.
- Work with the patient centered team to ensure assessment of patient's levels of insecurities in regards to food, transportation, and housing needs
- Support development and implementation of personalized treatment plans tailored to each patient's needs, incorporating evidence-based practices.
- Manage acute and chronic illnesses across various specialties such as geriatrics, behavioral health, and internal medicine.
- Coordinate care through case management, discharge planning, and utilization review to ensure seamless patient transitions across settings like outpatient clinics, hospitals, nursing homes, and assisted living facilities.
- Utilize electronic health record (EHR) systems such as Athena for accurate documentation of patient encounters and medical records.
- Provide health coaching and education to promote lifestyle modifications, medication adherence, and disease prevention strategies.
Skills
- Autonomous practice license is required
- Proven experience working with diverse populations including seniors, individuals with disabilities or developmental disabilities, and those requiring hospice or palliative care.
- Strong proficiency in EMR/EHR systems like Athenahealth; familiarity with CPT coding, ICD-10/ICD-9 coding standards is highly desirable.
- Expertise in managing complex cases involving infectious disease care, acute pain management, trauma (Level I/II Trauma Center experience), ICU or emergency medicine backgrounds preferred.
- Knowledge of HIPAA regulations ensuring confidentiality and security of patient information; experience with telehealth platforms is a plus.

Pay: $108,341.00 - $130,475.18 per year
