Overview
Kindred Community Cares is a new start-up multi-specialty clinic providing primary care and behavioral healthcare. We are preparing to relocate in order to expand our services. Join our dynamic healthcare team as a Family Nurse Practitioner (FNP), where you will play a vital role in delivering comprehensive, patient-centered care across diverse settings. You will support our efforts to address social determinants of health. You will utilize your advanced nursing skills to assess, diagnose, and manage a wide range of chronic health conditions, ensuring optimal outcomes for patients of all ages. Duties

  • Conduct thorough initial intake and follow-up physical examinations and patient assessments, including vital signs, medical history reviews, and physiological evaluations.
  • Work with the patient centered team to ensure assessment of patient's levels of insecurities in regards to food, transportation, and housing needs
  • Support development and implementation of personalized treatment plans tailored to each patient's needs, incorporating evidence-based practices.
  • Manage acute and chronic illnesses across various specialties such as geriatrics, behavioral health, and internal medicine.
  • Coordinate care through case management, discharge planning, and utilization review to ensure seamless patient transitions across settings like outpatient clinics, hospitals, nursing homes, and assisted living facilities.
  • Utilize electronic health record (EHR) systems such as Athena for accurate documentation of patient encounters and medical records.
  • Provide health coaching and education to promote lifestyle modifications, medication adherence, and disease prevention strategies.

Skills

  • Autonomous practice license is required
  • Proven experience working with diverse populations including seniors, individuals with disabilities or developmental disabilities, and those requiring hospice or palliative care.
  • Strong proficiency in EMR/EHR systems like Athenahealth; familiarity with CPT coding, ICD-10/ICD-9 coding standards is highly desirable.
  • Expertise in managing complex cases involving infectious disease care, acute pain management, trauma (Level I/II Trauma Center experience), ICU or emergency medicine backgrounds preferred.
  • Knowledge of HIPAA regulations ensuring confidentiality and security of patient information; experience with telehealth platforms is a plus.
  • Excellent communication skills for triage assessments, patient education, interdisciplinary collaboration, and documentation accuracy. Join us in making a meaningful difference through compassionate care delivered with energy and professionalism!

Pay: $108,341.00 - $130,475.18 per year
