Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility assistance
- Monitor and observe patients’ health status, recording vital signs and reporting changes promptly
- Support medication administration following medical protocols and documentation standards
- Provide companionship and emotional support to reduce feelings of isolation, especially for seniors or individuals with disabilities
- Operate Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to document care activities accurately
- Assist with household tasks such as light cleaning, laundry, meal preparation, and maintaining a safe environment
- Use specialized equipment like Hoyer lifts or catheters safely and effectively during patient transfers or care routines
- Follow HIPAA guidelines to ensure patient privacy and confidentiality at all times
- Collaborate with nursing staff, therapists, and family members to deliver comprehensive care plans
- Support clients in community settings or hospice environments, respecting their individual needs and preferences
Skills
- Proven experience in home health care, assisted living, or long-term care environments
- Strong knowledge of medical terminology, dementia care, Alzheimer’s care, and memory care practices
- Skilled in patient observation, vital signs measurement, medication administration, and medical documentation
- Familiarity with EMR/EHR systems for accurate record keeping and reporting
- Ability to assist with complex procedures such as catheter care or hospice support while maintaining safety standards
- Experience working with individuals with developmental disabilities or disabilities in general
- CPR certification and knowledge of emergency response procedures
- Compassionate caregiver with excellent communication skills and patience
- Ability to work independently while following detailed care plans and protocols
- Willingness to perform light housekeeping tasks including cleaning, laundry, and meal prep to support overall client well-being
