We are seeking a dedicated and highly skilled Registered Nurse (RN) to serve as a Visiting Nurse, providing comprehensive patient care in community settings. Essential Duties and Responsibilities
· Perform comprehensive nursing assessments during home visits, including review of vital signs, medications, functional status, safety risks, and changes in condition.
· Develop, implement, and update individualized plans of care in collaboration with the patient, family/caregiver, physician, and interdisciplinary care team.
· Provide skilled nursing services, including medication administration and reconciliation, wound care, disease management education, injections, IV therapy support when applicable, catheter care, and other treatments within scope of practice.
· Monitor patient progress, identify changes in condition, and communicate concerns promptly to the physician, clinical supervisor, and care team.
· Educate patients and caregivers on medications, treatment plans, symptom management, fall prevention, infection control, nutrition, and strategies to support independence at home.
· Coordinate care with physicians, therapists, social workers, home health aides, and other providers to ensure continuity and quality of care.
· Complete accurate, timely, and compliant clinical documentation, including visit notes, care plan updates, OASIS assessments when required, and discharge summaries.
· Participate in case conferences, quality improvement activities, and agency meetings as assigned.
· Follow infection prevention, patient privacy, safety, and emergency response policies at all times.
