Role Description The Certified Home Health Aide will provide direct, hands-on care and support to clients in their homes, ensuring comfort, safety, and well-being. Day-to-day responsibilities include assisting with activities of daily living such as bathing, dressing, grooming, toileting, and mobility, as well as helping with light housekeeping, meal preparation, and medication reminders as directed. The Home Health Aide will observe and report changes in clients’ conditions, follow care plans, and collaborate with nurses and other care team members. This role also involves offering companionship, emotional support, and respectful communication to promote a positive home environment. This is a part-time, on-site role based in Union, NJ.

Qualifications

  • Demonstrated Home Care experience and familiarity with in-home support routines.
  • Ability to communicate clearly, respectfully, and professionally with clients, families, and care team members.
  • Proficiency in assisting with Activities of Daily Living, including bathing, dressing, grooming, toileting, and mobility.
  • Strong Patient Care skills, including monitoring client status, following care plans, and reporting concerns.
  • Competence in providing Personal Care with sensitivity, privacy, and cultural awareness.
  • Current Home Health Aide certification in New Jersey and compliance with state requirements.
  • Reliability in attendance, time management, and adherence to safety and infection control practices.
  • Ability to lift, transfer, and support clients safely, as needed, and to work in various home environments.
  • High school diploma or equivalent preferred; prior experience in home health or related care settings is beneficial.
