We are seeking a dedicated and knowledgeable Nutritionist to join our healthcare team. This role is focused primarily on assessing nutritional needs, building individualized nutrition plans, and supporting care strategies through evidence-based recommendations.
The Nutritionist will work remotely to review client information, create tailored care plans, support wellness outcomes, and maintain accurate documentation. This role is less patient-facing and more focused on plan development, nutritional analysis, and care planning support.
Core Responsibilities
- Patient Assessment & Care Planning
- Conduct comprehensive nutritional assessments to identify patient needs
- Develop individualized nutrition care plans based on health history, goals, and medical conditions
- Review and update care plans as patient needs evolve
- Nutrition Plan Development
- Develop customized nutrition plans aligned with client health history, goals, and medical needs
- Build structured recommendations to support wellness, disease management, and healthy lifestyle habits
- Adjust nutrition strategies based on updated information and outcomes
- Monitoring & Progress Tracking
- Monitor patient outcomes and evaluate the effectiveness of nutrition interventions
- Adjust care plans and recommendations as needed
- Maintain accurate documentation of patient progress
- Documentation & Collaboration
- Maintain accurate nutrition documentation and care plan records
- Collaborate with internal teams and healthcare providers when appropriate
- Support consistency in care planning and nutrition-related recommendations
- Bachelor’s degree or higher in Nutrition, Dietetics, or related field
- Registered Dietitian (RD) credential is a plus
- Strong understanding of nutrition science, physiology, and dietary planning
- Excellent communication and documentation skills
- Comfortable working independently in a remote environment
- Experience in healthcare, wellness, or clinical nutrition preferred
- Timely completion of patient assessments
- Accurate and compliant documentation
- Consistent completion of care plans and documentation
- Improved patient outcomes through nutrition intervention
- Strong adherence to care plan standards and clinical guidelines
