MISSION

Kheir’s mission is to increase and provide access to culturally and linguistically-sensitive quality primary healthcare and human services support to the underserved and uninsured residents of Southern California.

CLINIC REGISTERED NURSE ASSIGNMENT SUMMARY

We are seeking a dynamic Registered Nurse (RN) to join our Clinic. The RN plays a vital role in delivering high-quality, patient-centered care to underserved and traditionally marginalized communities. In addition to core primary care responsibilities, this RN will actively participate in the development and delivery of Kheir’s Lifestyle Medicine program, which integrates body composition analysis, extended in-person consultations, and structured documentation to support prevention and chronic disease management. The RN works collaboratively with providers, medical assistants, and other members of the care team to ensure comprehensive and culturally competent healthcare services in accordance with the mission and values of the health center. As we face growing health and social inequities, we need passionate, community-inspired professionals more than ever.

CLINIC REGISTERED NURSE ESSENTIAL JOB FUNCTIONS

Clinical

  • Provide direct nursing care to patients, including assessments, triage, and health education
  • Administer medications, immunizations, and treatments as prescribed by providers
  • Support providers during examinations and minor procedures
  • Document all patient interactions accurately in the Electronic Health Record (EHR)
  • Participate in quality improvement initiatives and population health programs
  • Educate patients and families on health conditions, treatment plans, and preventive care
  • Ensure compliance with clinical protocols, infection control standards, and regulatory requirements
  • Collaborate with interdisciplinary teams to address social determinants of health
  • Respond to medical emergencies and perform CPR or other life-saving interventions as needed.
  • Manage the Clinic Nurse Line and provide guidance to patients
  • Other duties as assigned.

Lifestyle Medicine Program

  • Provide Lifestyle Medicine visits to targeted patients, incorporating extended consultations focused on nutrition, physical activity, sleep, stress, and other lifestyle factors
  • Perform and interpret body composition assessments using the body composition analyzer, integrating results into lifestyle coaching and patient education
  • Use structured Lifestyle Medicine documentation tools along with eCW to capture initial assessments, individualized goals, and ongoing progress.
  • Collaborate with providers to co-create and update personalized action plans and scorecards that patients can follow between visits
  • Track and monitor patient progress over time, preparing information for follow-up visits and ensuring continuity of care
  • Coordinate with existing staff to ensure smooth scheduling, communication, and integration of Lifestyle Medicine visits into clinic operations

Administrative

  • Participate in staff meetings, huddles, and case conferences as needed
  • Contribute to internal reporting and data collection related to Lifestyle Medicine program outcomes

CLINIC REGISTERED NURSE MINIMUM QUALIFICATIONS

  • Current and unrestricted RN license in the state of California
  • Associate’s or Bachelor’s degree in Nursing from an accredited program
  • Current and Active BLS certification
  • Minimum 2 years of clinical nursing experience (preferably in primary care, community health, or public health)
  • Subject to a background check and sign confidentiality disclosure

Preferred:

  • Interest or training in Lifestyle Medicine, health coaching, nutrition, or related fields
  • Experience working in an FQHC or with underserved populations
  • Bilingual or multilingual skills
  • Familiarity with EHR systems (e.g., eClinicalWorks)

WORKING CONDITIONS/PHYSICAL REQUIREMENTS:

This is primarily a clinic position that requires frequent walking, bending, reaching, stooping, lifting and moving of office materials weighing 25 pounds or less. May involve assisting patients with mobility or positioning. Must be able to respond quickly to emergency situations.
