Keystone Post Acute is a 65-bed community that offers exceptional skilled nursing care for seniors recovering from surgery, rehabilitating from a specific injury or trauma, or those in need of high acuity nursing care. Our highly trained team of nurses are on-site 24 hours a day, 7 days a week, providing general and specialized care to ensure a swift and complete recovery
Shift Availability
- Full Time PM-Shift: 2:00 PM - 10:30 PM
- Per Diem - All Shifts
Responsibilities
- Conduct thorough patient assessments, including vital signs, physical examinations, and medical history documentation using electronic health record Point Click Care.
- Administer medications and injections accurately while adhering to aseptic techniques and HIPAA regulations to ensure patient safety and confidentiality.
- Perform specialized procedures including IV insertion, suturing, catheterization, tube feeding management, and wound care in accordance with established protocols.
- Collaborate with physicians and healthcare teams to develop individualized care plans for patients with complex conditions such as trauma, infectious diseases, or chronic illnesses.
- Ensure compliance with infection control standards and participate in quality improvement initiatives aligned with NCQA standards.
- Document all patient interactions thoroughly in medical records using Point Click Care for accurate case management and billing purposes.
- Provide direct nursing care to residents, including assessments, treatments, and medication administration.
- Complete and document comprehensive nursing assessments in accordance with Title 22, OBRA, and facility policy.
- Monitor residents’ conditions and communicate changes promptly to physicians, families, and the interdisciplinary team.
- Supervise and support LVNs, CNAs, and other clinical personnel to ensure quality care delivery.
- Ensure proper infection control practices and follow all CDPH, CMS, and facility regulations.
- Respond to emergencies and perform clinical interventions within scope of practice.
- Maintain accurate and timely nursing documentation in the EMR.
- Uphold resident rights, dignity, and safety at all times.
Qualifications
- Current California RN license in good standing.
- Experience in long-term care or post-acute/SNF setting preferred but not required—new grads welcome!
- Strong assessment, communication, and leadership skills.
- Ability to work collaboratively with a multidisciplinary team.
- Knowledge of Title 22, OBRA guidelines, infection control standards, and SNF regulatory requirements preferred.
- Demonstrated expertise in handling complex cases involving acute pain management, trauma medicine, infectious disease care, or geriatrics such as memory care or Alzheimer's/dementia care.
- Proficiency in performing diagnostic evaluations utilizing laboratory testing, sonography, or medical imaging techniques like mammography or gastroscopy.
- Familiarity with coding systems such as ICD-10/ICD-9 and CPT coding for accurate documentation and billing processes.
- Knowledge of nursing procedures including phlebotomy, specimen collection/processing, sterile processing techniques, and ostomy care.
- Prior experience working within compliance frameworks such as HIPAA regulations and NCQA standards is highly desirable.
- Additional qualifications include certification in basic life support (BLS), advanced cardiovascular life support (ACLS), or specialized training in hospice & palliative medicine or occupational health.
