Qualifications
- Strong foundation in Nursing and Medicine with a registered nurse (RN) license, and relevant clinical experience.
- Proficient in Case Management, discharge planning, and patient care coordination.
- Experience in Utilization Management, including the ability to assess care needs and align them with resources effectively.
- Excellent communication, organizational, and problem-solving skills.
- Compassionate and empathetic with the ability to support patients and families in emotional and challenging situations.
- Knowledge of hospice care guidelines and prior experience working in hospice or palliative care is a plus.
