What You’ll Do

As an RN Health Coach, you’ll support family caregivers through a combination of:

• In-home visits

• Monthly coaching and support calls

• Care planning and health education

• Documentation and compliance tracking

• Building trusted relationships with families

You’ll play a major role in helping elderly and disabled clients remain safely at home while supporting the family members caring for them.

Qualifications

• Active Georgia Registered Nurse (RN) license

• Strong communication and people skills

• Compassion for elderly and disabled populations

• Reliable transportation and ability to travel within assigned territory

• Comfortable using technology and documentation systems

• Home care, case management, or elder care experience is a plus, but not required

• $65,000–$85,000 annual salary
