Responsibilities
- Assist residents or clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support.
- Monitor and observe patients’ conditions closely, noting any changes in health status or behavior to ensure prompt response and appropriate documentation.
- Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary.
- Administer medications accurately following care plans and HIPAA privacy regulations to ensure patient safety and confidentiality.
- Support residents with mobility using equipment such as Hoyer lifts or resident’s , ensuring safe transfers and positioning.
- Conduct routine patient monitoring by taking vital signs such as blood pressure, pulse, temperature, and respiration rates.
- Maintain a clean and safe environment through regular cleaning duties, laundry assistance, and organization of personal belongings.
- Follow established care plans meticulously while collaborating with healthcare professionals to deliver consistent, high-quality care.
- Engage residents in social activities or recreational programs to promote mental stimulation and emotional well-being.
- Document all care activities accurately in compliance with HIPAA standards and facility protocols.
- Support hospice or palliative care needs with sensitivity, respecting each individual’s dignity and preferences.
- Work effectively within team settings that include nursing staff, social workers, and other caregivers to coordinate comprehensive support services.
Requirements
- Proven experience in home health, assisted living, nursing home environments, or group homes is highly preferred.
- Knowledge of patient observation techniques and ability to recognize signs of health deterioration or distress.
- Certification in CPR (Cardiopulmonary Resuscitation) and First Aid is required; additional certifications such as HIPAA training are advantageous.
- Familiarity with Alzheimer’s care, dementia care, memory care strategies, autism support, or working with people with developmental disabilities is desirable.
- Ability to perform heavy lifting safely using equipment like Hoyer lifts or resident’s when necessary for patient transfers.
- Strong communication skills for effective interaction with residents, families, and healthcare teams.
- Demonstrated ability to follow detailed care plans accurately while maintaining flexibility for changing needs.
- Valid driver’s license may be required depending on the role’s scope involving community-based services.
