Summary: The Clinical Quality Manager will facilitate the development, coordination, implementation and evaluation of patient care activities and clinical quality improvement initiatives within each medical team. Specifically, the position will focus on areas such as efficient use of EHR, and other clinical outcome measures, Patient Centered Medical Home requirements, chronic disease management, and other clinical quality related activities.

Essential Duties:

  • Assists the Project Director and Clinical Director with Quality Improvement (QI) efforts within the clinic. Assists the Project Director in leading audits and peer review processes, also providing support in the gathering and interpretation of relevant clinical data. Assists the Medical Director in holding Quality Improvement Committee meetings.
  • Clinical Quality Manager is accountable for implementation and sustainability of the patient centered medical home model. This individual will lead the recognition and transformation processes. The Clinical Quality Manager will be the primary point of contact for NCQA as well as staff with regards to PCMH initiatives. The Clinical Quality Manager will lead ongoing reporting and sustainability plans and has an integral role on the QI team. leads, supports, and sets the tone for how practice will function as a Medical Home. Acquires and maintains expert knowledge of NCQA Patient Centered Medical Home (PCMH) standards, policies, processes, and application/renewal requirements.
  • Serve as on-site Risk Manager for the program. Updates annual Risk Management Plan and goals. Assesses, monitors, and identifies potential medical malpractice risk through risk analysis tools. Works with Agency Risk Manager to conduct Physician Practice Risk Management Assessments, Facility Safety Inspections, recommend and complete corrective action plans. Ensures clinic staff participate in annual risk management trainings as needed and/or required. Serves as a representative on the QI committee to report identified risks.
  • Performs all duties in support of successful EHR/EPM implementation including assistance with support tickets. Analyzes EHR problems, determines cause, and initiates corrective action.
  • Strategizes better health outcomes using continuous improvement processes. Involves and trains staff on updated processes and monitors sustainability.
  • Monitors clinical outcomes on UDS (6a, 6b, 7), MU, and HQM.
  • Leads QI subcommittee meetings, updating staff on QI changes and QI performance.
  • Monitors all patients receiving chronic disease management and/or case management for improvement in key indicators.
  • Tracks and assists Project Director in establishing contracts with specialty providers to ensure compliance with grant requirements (Form 5A).
  • Assists in rooming patients as needed (collecting medical/social history, vitals, reason for visit, etc.) including completing nursing standing orders as authorized by the Providers. Interprets and reports observations, in keeping with sound nursing/CMA practices. Maintains proper clinical Electronic Health Records and reports, using problem-oriented medical records which can be audited for accountability.
  • Attends mandatory staff meetings and trainings as necessary.
  • May delegate duties and responsibilities, but no portion of accountability for results and overall performance may be relinquished.
  • Performs all other duties, as assigned by supervisor.

Education, Experience & Training:

Must be a licensed RN, LPN, or CMA. CPR certification required. Bachelor’s degree in healthcare administration, public health, social services, or other health related field along with relevant experience may be considered. Three (3) years minimum experience with quality improvement oversight in a health care setting strongly preferred along with administrative and management skills in personnel, clinical measures, marketing and advocacy.

Must demonstrate commitment and sensitivity to the needs of an underserved homeless population. Prior experience with a Federally Qualified Health Center (FQHC) or Rural Health Clinic (RHC) helpful. Must be proficient in most computer applications, including Microsoft Office and NextGen/EHR, and have good oral and written communication skills.

Position requires occasional or frequent moderate physical activity. While performing the duties of this job, the employee is regularly required to talk or hear. The employee is occasionally required to stand; walk, climb, balance, stoop, kneel, crouch, crawl, and use hands to manipulate, handle or feel; and reach with hands and arms. The employee is occasionally required to lift/and or exert force up to 50 pounds.

Other Requirements:

Must have a valid driver’s license and the flexibility and willingness to change schedules and duties as needed according to staff availability. Must be willing to travel the service area as needed. Travel is primarily local or within the service area during the business day, although some out-of-area and overnight travel may be expected.

Agency policy regarding drug and alcohol testing and criminal background check applies to this position. Must maintain a high level of professionalism, demonstrated by a positive attitude, ability to function as a team member, and a commitment to the overall mission of the agency.
