Role Description
This is a full-time, on-site role for a Registered Nurse Assessment Coordinator (RNAC) located in Wilmington, DE. The RNAC will oversee and manage the completion of resident assessments as part of the MDS (Minimum Data Set) process, ensuring compliance with state and federal regulations. Responsibilities include coordinating care plans, monitoring resident progress, collaborating with interdisciplinary teams, and maintaining accurate documentation. The Registered Nurse Assessment Coordinator will also serve as a resource for staff and participate in quality improvement initiatives to enhance care practices.
Qualifications
- Comprehensive knowledge of resident assessments, care plans, and MDS processes
- Strong organizational, analytical, and documentation skills
- Effective communication and collaboration skills with interdisciplinary teams
- Experience with regulatory compliance and familiarity with federal/state regulations in skilled nursing settings
- Active Registered Nurse (RN) license in the State of Delaware
- Bachelor’s degree in Nursing (BSN) preferred or equivalent clinical experience
- Proficiency with electronic medical records and computer systems
