POSITION SUMMARY
The Patient Access Representative in the LTAC setting is responsible for coordinating patient admissions and managing appointment scheduling with a high level of accuracy and compassion. This role ensures all admissions packets are complete and compliant with regulatory standards, verifies patient insurance and authorizations, and serves as a vital point of contact between patients, families, providers and referring facilities. This position requires strong interpersonal and administrative skills to support medically complex, long-stay patients.
KEY RESPONSIBILITIES
- Coordinate and schedule patient appointments, evaluations, and admissions in alignment with clinical availability and patient needs, and track in EMR
- Obtain follow up records from appointments, scan into EMR, coordinate with MD to schedule follow up appointments.
- Prepare and complete comprehensive admissions packets, including consents, face sheets, insurance verification, and supporting documentation.
- Communicate with referring hospitals, SNFs, physicians, and case managers to gather and verify clinical and demographic information.
- Verify insurance benefits and obtain prior authorizations as needed for LTAC admission and continued stay.
- Accurately enter/scan patient data into the EHR system and maintain up-to-date records.
- Greet and assist patients and families, providing information and reassurance during the intake and admission process, and provide family tours.
- Ensure compliance with LTAC admission protocols and CMS/regulatory requirements.
- Coordinate with case management, nursing, and therapy teams to facilitate smooth transitions into the facility.

POSITION QUALIFICATIONS
MINIMUM QUALIFICATIONS:
- High school diploma or GED required; associate degree or healthcare certification preferred
- Minimum 1-2 years of experience in hospital admissions, patient access, or healthcare scheduling; LTAC, acute care, or rehabilitation hospital experience strongly preferred.
- Knowledge of LTAC-specific admissions criteria, insurance types (Medicare, Medicaid, Commercial), and authorization processes.
- Familiarity with EHR systems; experience with systems like WellSky, Epic, or Cerner is a plus.
- Strong communication, customer service, and problem-solving skills.
- Detail-oriented with the ability to multitask in a high-volume environment
- Bilingual in English/Spanish or other languages is a plus.
