KVNA's patient population is growing, therapy services are in demand to keep individuals safe and able to function in their home environment. We are seeking a full-time physical therapist to join our extraordinary team of clinicians. PHYSICAL THERAPIST—Home Health Care Home health care—patient and family centered care, offers personalized care tailored to the needs of the individual. Home care allows for dignity, independence and maximum comfort for the patient and family, leading to a higher satisfaction with daily life. As a KVNA Physical Therapist, you will make a real-time difference in the patient's recovery and quality of life. Physical Therapists work with patients to improve their movement and manage their pain. A home care physical therapist assumes responsibility for the ongoing interdisciplinary assessment and development of each patient’s individual plan of care. The home care physical therapist will work in partnership with the patient’s family, caregivers and physician to assist the patient in reaching their recovery goals. Duties and Responsibilities include: Assessing and evaluating therapeutic/rehabilitative/functional status to develop the total plan of care Assessing muscle strength, mobility, gait, ROM—potential for rehab Initiating physical therapy program and instruct Instructing patients and families/caregivers in the use and care of therapeutic appliances Reporting to physician the patient’s response to treatment of changes in condition Identifying patient and family/caregiver needs for other home health services and refers as necessary. Communicating with the physician responsible for the home health plan of care and other practitioners related to the patient’s current home health plan of care Participating in discharge planning for the patient. Supervising Physical Therapist Assistants according to policy and procedure and state regulations. Submission of thorough clinical notes and progress summaries based on achievement of goals. Participating in the agency’s quality assessment and performance improvement program. Job Requirements: Possess a degree from a baccalaureate or master’s program in physical therapy approved by an accredited organization Licensed to practice as a physical therapist in the State of Wisconsin Two years of appropriate experience as a physical therapist; community or home health experience is preferred Licensed driver with an insured automobile in good working order Current CPR certification The employer for this position is stated in the job posting.
