Pharmacist Home Infusion Per Diem

This is a per diem position.

Job Summary:

We are seeking a dedicated pharmacist to join our team, specializing in home infusion. This is a non-retail clinical pharmacist position, and home infusion experience is not necessary. Some experience in sterile compounding is desired, and we are willing to provide training to the right candidate.

In this role, you will be responsible for dispensing medications and drugs, providing clinical consultation, and ensuring the highest standards of patient care.

Responsibilities Include:

• Coordinate patient admissions with the healthcare team, including physicians, pharmacists, home health agencies, and case managers.
• Provides clinical consultation for I.V. therapies, dosage calculations, nutritional support, pharmacokinetics, and drug information as needed.
• Assume the role of Pharmacist in Charge (PIC) in the absence of the Homecare Pharmacy Department PIC.
• Oversees staff and assists customers with medication selection, equipment, and supplies in person and by telephone.
• Ensure patient confidentiality and rights are upheld.
• Perform other work-related duties as assigned by the supervisor.

Requirements:

• Bachelor of Science in Pharmacy or PharmD Degree
• Current Maine Registered Pharmacist License
