What You Will Do
- Float between Skilled Nursing and Assisted Living neighborhoods based on resident care and staffing needs
- Administer medications and treatments in accordance with physician orders and facility policies
- Monitor residents' conditions and communicate changes to physicians, supervisors, and care team members
- Assist with resident assessments, care planning, and MDS-related documentation as assigned
- Perform direct resident care, including activities of daily living (ADLs) such as bathing, dressing, grooming, toileting, mobility assistance, and feeding assistance
- Provide restorative nursing support to help residents maintain or improve functional abilities
- Assist with admissions, transfers, and discharges
- Complete accurate and timely documentation in compliance with regulatory requirements
- Participate in audits, chart reviews, infection prevention activities, and quality assurance initiatives
- Support residents and families through compassionate communication and resident-centered care
- Respond appropriately to emergencies and changing resident care needs
What You Bring
- Current Licensed Practical Nurse (LPN) license in the Commonwealth of Virginia
- Graduation from an approved Licensed Practical Nursing program
- Previous experience in long-term care, skilled nursing, assisted living, or restorative nursing preferred
- Excellent communication, organizational, and time management skills
- Strong critical thinking and problem-solving abilities
- Ability to work independently and collaboratively within an interdisciplinary team
- Flexibility to work across multiple care settings and adapt to changing assignments
