Kendal~Crosslands Communities is seeking a compassionate, detail-oriented Health Services Admissions Liaison to support admissions for the Health Centers at both Crosslands and Kendal at Longwood. This role serves as a primary point of contact for prospective residents, families, hospitals, referral sources, and internal teams, helping ensure a smooth, responsive, and professional admission experience.
This is a part-time position, working 20 hours per week.
Position Summary:
The Health Services Admissions Liaison manages the admissions process for short-term and long-term per diem admissions to skilled nursing and personal care settings. The liaison coordinates referrals, assessments, documentation, insurance verification, and communication among residents, families, referral partners, and the interdisciplinary care team.
Qualifications:
• Bachelor’s degree and/or a minimum of three years of healthcare experience.
• Knowledge of admission requirements for skilled nursing facilities and personal care homes.
• Excellent verbal and written communication skills with a strong customer service orientation.
• Ability to multi-task, prioritize, and maintain accuracy in a fast-paced environment.
• Demonstrated ability to work collaboratively with residents, families, referral sources, healthcare professionals, and internal teams.
• Self-directed, proactive, and able to take initiative.
• Experience with healthcare admissions, referral coordination, insurance verification, or medical administrative support preferred.
