Duties
- Conduct thorough patient assessments, including medical history review and physiological evaluations, to determine individual needs and develop personalized care plans
- Coordinate discharge planning by collaborating with multidisciplinary teams, community resources, and families to ensure safe and effective transitions from inpatient to outpatient or post-acute care settings
- Manage utilization review processes, ensuring appropriate level of care and adherence to clinical guidelines in compliance with HIPAA regulations and medical records standards
- Facilitate communication between patients, families, healthcare providers, and community resources to support patient-centered care and social work interventions
- Monitor patient progress through electronic health records (EHR) management, documenting clinical case management activities accurately and efficiently
- Prioritize tasks effectively in high-pressure situations such as Level I Trauma Centers or ICU environments while maintaining exceptional patient service standards
- Provide behavioral health support and mental health counseling for patients with complex psychosocial needs, including those with substance use disorders or disabilities
Requirements
- Proven experience working in hospital environments, especially within Level II or Level I trauma centers, ICU units, or emergency medicine settings
- Strong background in acute care, hospital medicine, pediatrics, or behavioral health services with familiarity in PICU (Pediatric Intensive Care Unit) experience preferred
- Knowledge of medical terminology, physiology, diagnostic evaluation techniques, and clinical case management practices
- Experience working with diverse populations including children, individuals with disabilities or mental health conditions, and those from varied cultural backgrounds
- Proficiency in electronic health records (EHR) systems and familiarity with HIPAA compliance standards for patient privacy and data security
- Certification in Basic Life Support (BLS), CPR, and addiction counseling is highly desirable
- Excellent communication skills for client interaction and working closely with families and multidisciplinary teams
- Ability to prioritize tasks efficiently in fast-paced hospital settings while maintaining a focus on patient safety and service excellence
