Key Responsibilities
· Conduct comprehensive nutrition assessments across assigned facilities
· Develop and implement individualized nutrition care plans based on clinical guidelines and patient needs
· Provide nutrition interventions including weight management, therapeutic diets, and enteral nutrition support
· Collaborate with interdisciplinary teams to support patient care and improve outcomes
· Support dietary department operations including regulatory compliance, food safety, and sanitation standards
· Participate in care planning, quality improvement, and survey readiness activities
· Provide education and guidance to staff, patients, and caregivers
· Document all assessments and interventions within EMR systems accurately and efficiently
· Maintain current knowledge of clinical nutrition practices across long-term care, acute care, and related settings
