What You’ll Do
- Conduct client assessments and develop thoughtful, personalized care plans
- Provide clinical oversight across private duty home care and four RCFE (board & care) homes
- Support and guide caregivers through training, mentoring, and ongoing development
- Foster a culture of consistency, compassion, and accountability within the care team
- Collaborate closely with families and referral partners to ensure a supportive, high-quality experience

What You Bring
- Active California RN license in good standing, and without restrictions
- 10 years of experience in hospital, skilled nursing, home health, and/or hospice settings
- Strong clinical judgment paired with a calm, thoughtful approach
- A natural ability to guide, support, and develop others
- High emotional intelligence—you connect easily while maintaining professional boundaries
- A collaborative, patient, and solutions-oriented mindset
Role Requirements
- Flexibility for occasional evening or weekend assessments
- Valid driver’s license, car insurance, and ability to travel locally
- Comfort balancing clinical oversight, team support, and relationship-building
Salary : $120,000 - $150,000
